Supplement to Consumer Health Data Privacy Policy for Nevada Consumers

1. The Categories of Consumer Health Data We Collect and How We Use Them.

We collect the below categories of Consumer Health Data from Consumers.

  • Individual health conditions, treatment, diseases, or diagnosis;
  • Social, psychological, behavioral, and medical interventions;
  • Health-related surgeries or procedures;
  • Use of medications;
  • Bodily functions, vital signs, symptoms, or measurements related to health;
  • Diagnoses or diagnostic testing, treatment, or medication;
  • Gender-affirming care information;
  • Reproductive or sexual health information;
  • Biometric data;
  • Genetic data;
  • Precise location information that could reasonably indicate a Consumer’s attempt to acquire or receive health services or supplies;
  • Data that identifies a Consumer seeking health care services;
  • Health-related data that has been derived, inferred, or extrapolated from the above.

Examples of Consumer Health Data we may collect include health conditions or diagnoses shared by you, medication-related information, service needs, and other health-related information you choose to provide in connection with Swanson Health’s services.

We use the above-mentioned categories of Consumer Health Data for the following purposes. To:

It supplements the Consumer Health Data Privacy Policy and applies solely to information or practices that are subject to Nevada’s Consumer Health Data privacy law. Capitalized terms used but not otherwise defined herein shall have the meanings afforded to them in applicable Consumer Health Data privacy laws, our General Privacy Policy, or our Terms of Service.

Collection, Use, and Processing of Consumer Health Data. We collect, use, process, and disclose Consumer Health Data for the purposes and in the manners described in Section 1 and Section 2 of our Consumer Health Data Privacy Policy.

Disclosure of Consumer Health Data. We may disclose your Consumer Health Data in the manner and for the purposes described in Section 1 and Section 3 of the Consumer Health Data Privacy Policy with the categories of Third Parties and Affiliates described in Section 4 of our Consumer Health Data Privacy Policy.

Exercising Your Privacy Rights and Review & Revision of Your Consumer Health Data. Applicable law provides eligible Consumers with specific rights regarding their Consumer Health Data. For a description of these rights, as well as instructions on how to exercise them, refer to Section 5 of our Consumer Health Data Privacy Policy. If you would like to review and/or revise your Consumer Health Data, you may submit a request to us via any of the methods listed in Section 5 of our Consumer Health Data Privacy Policy. We will respond to your requests to exercise your rights in accordance with applicable law, but in any case no later than 45 days after receiving your request. We may extend this period up to 45 days only where doing so is permitted under applicable law.

Changes to this Supplement. From time to time, we may change this Supplement. We will notify you in the same manner as set forth in Section 6 of the Consumer Health Data Privacy Policy. Your access to and/or use of the Website after such change will be deemed acceptance of the new Supplement.
